Applications and Fees
The Commission on Gaming has converted its forms to the
ADOBE .PDF format (Portable Document Format) which is both device and resolution
independent. The printed version of these PDF documents will be similar
regardless of the device used to print. In
order to view or print the Adobe Acrobat forms, you must download the free
ADOBE Acrobat Reader
for Windows, DOS or for Macintosh. For best viewing results, we recommend
using Adobe Acrobat Reader 7.0.
NOTE: All applicants must submit
completed fingerprint cards with their application to the South Dakota
Commission on Gaming. The cards must be obtained from the South Dakota
Commission on Gaming, Deadwood office at (605) 578-3074.
Deadwood
Gaming Licenses
The Commission may issue six types of gaming licenses as
follows:
Slot Machine Manufacturer or Distributor License Application
Renewal Application
Any person or distributor who designs, assembles, fabricates,
produces, constructs, sells, leases, or who otherwise prepares a product or a
component part of a slot machine, other than tables or cabinetry must complete
this application. The application fee is $5,000 and the license fee is
$1,000 with a fiscal year (July through June) renewal fee of $250.
Operator License Application

Application
Any person who places slot machines in the person’s own business
premises must complete this application. The license fee is $1,000 with
a fiscal year (July through June) renewal fee of $200.
Route

Application
Any person who, individually or jointly, pursuant to an agreement
whereby consideration is paid for the right to place slot machines or gaming
tables, engages in the business of placing and operating slot machines or
gaming tables within the City of Deadwood must complete this application. You
must have an Operator License to obtain Route Operator License.
The application fee is $1,000 and the license fee is $1,000 with a fiscal year
(July through June) renewal fee of $200.
Retail License Application

Any licensee who maintains gaming at the licensee’s place of business
within the City of Deadwood for use and operation by the public must complete
this application. The application fee is $250 and the license fee
is $250 with a fiscal year (July through June) renewal fee of $100.
Key Employee
License Application
Renewal Application
Any executive, employee, or agent of a gaming licensee having the power to
exercise a significant influence over decisions concerning any part of the
operation of a gaming licensee must complete this application. The
application fee is $150 and the license fee is $150 with a calendar year
(January through December) renewal fee of $75.
Support License Application

A person licensed by the commission who is working or who is about to work
for an operator or retailer as a card dealer, slot machine technician, floor
supervisor, cashier, shill, proposition player, slot route runner, pit boss, a
member of a count team, computer monitor for progressive links, or in any
other way directly related to gaming must complete this application. The
application fee is $50 and the license fee is $50 with a calendar year
(January through December) renewal fee of $25.

Live Racing and Simulcast Licenses
Hub
Key
License Application
Any executive, employee, or agent of a Multi-Jurisdictional Wagering Hub in
South Dakota having the power to exercise a significant influence over decisions
concerning any part of the hub operation must complete this application. The
application fee is $50 with an annual renewal fee of $25 by December 31st
of each year.
Hub Employee License Application
Used for licensing of
pari-mutuel clerks at a Multi-Jurisdictional Wagering Hub or persons in any
other capacity directly relate to these hub locations must complete this
application. The application fee is $35 with an annual renewal fee of $20 by
December 31st of each year.

entity who wishes to be licensed as a Multi-Jurisdictional Wagering Hub in South
Dakota must complete this application.
Simulcast Provider License Application

entity providing a signal from another licensed racing jurisdiction to South
Dakota must complete this form.
Simulcast Site Application
Renewal Application
A location where a signal from another licensed racing jurisdiction to South
Dakota is received from a licensed simulcast provider must complete this form.
Simulcast Pari-Mutuel License
Application

Used for licensing of pari-mutuel clerks at
simulcast sites.
Note: If you have a valid
Deadwood support license, you are not required to obtain a Simulcast
Pari-Mutuel License.
Multiple Category Live Racing
